Tamar Braxton made an epic return to "Dancing With The Stars" last week after being hospitalized for a serious medical condition. At the time, she explained that upon returning to the hospital after the show, doctors realized it wasn't pneumonia she had but pulmonary embolisms, which are blood clots, in both of her lungs, as HNGN previously reported. Because of the health scare, Braxton had to eliminate herself from the competition, and after more than a week in the hospital, she is finally well enough to go home.

On Wednesday, the 38-year-old singer revealed the exciting news with an Instagram video of herself being wheeled out of the hospital. "Guess who's leaving the hospital!" the "Real" co-host says in the video. "Look at GOD!!" she wrote. "So blessed. Thank you for all your prayers. I [heart] you all so much."

When Braxton realized the condition would no longer allow her to stay in the competition, she was extremely upset and felt horrible that she had to be the reason her partner Val Chmerkovskiy could not continue. "With a heavy heart I regret to inform you that my season of #dwts has come to an end," she captioned a photo in the hospital last week. "As you all know I will be the first to always encourage anyone to push through any obstacles that comes along one's way. But in this case, my health is my current obstacle...I love you and thank you ALL for your support. P.S. big hugs and kisses to my boo @iamvalc for being beyond a friend, brother and the BEST partner EVER."

"Now that's the Tamar I know. The one I call my friend and after tonight will forever call my family," Chmerkovskiy captioned a happy selfie with his partner. "I love you with all my heart, and will always have your back." "All season long you impressed me with your honesty and heart, we became family," he captioned another shot of the two. "I wish you Health and am excited to be there to see your star shine for a very long time."
